Sports participation forms are now being accepted for the 2026 SPRING Sports Season. Forms for this upcoming season are due by FEBRUARY 13, 2026.

If you need to complete a Physician Physical Packet, only submit the Medical Eligibility Form (first page of packet) and be advised that forms can take a minimum of two weeks to process before the student-athlete is cleared to participate.
